Masonite* Instructions
Step 1 Call 1-800-323-4591 ask for a claim form to be sent to your home. Their office hours are 7:30pm to 3:30pm CST. It will take 7-10 days to receive your claim form. Once you receive it log back on here and continue to step 2.
Masonite will require the following for proof of ownership, siding and year built.
a. Paid tax receipt
b. Year built card (proof of year built)
c. 15 pictures of your home. (specific order of pictures is listed on your claim form)
d. For proof of siding you may take a picture of the logo on the back side of your siding
in your home. If you cant find and exposed peace then you will need a 6 inch by 6 inch sample cut.
